From the comfort and convenience of your home or workplace, you can chat with Dr. White and our Treatment Coordinator about what you’re hoping to accomplish and also discuss his recommendations for your treatment. There are just a few photos Dr. White needs to review beforehand in order to prescribe the best plan for you.

PREPARE FOR YOUR ZOOM CONSULTATION IN 3 EASY STEPS:

1. Click on the Zoom Consultations tab below and submit a request with a preferred day and time for your Zoom consultation. We will contact you to confirm it!
2. Watch our quick video titled Photo Instructions that will show you exactly how to take the photos and where to send them—it can all be done using your smartphone!
3. Submit a digital Health History Form (the forms can be found under the New Patients tab). Doing this will not only let us know of any pertinent health and medical information, but it will also give us an opportunity to look into your insurance coverage before your appointment so we can present you an accurate financial summary.
